Is the monitoring of the quality of US health care adequate?

As of today, there are well over 45 million Americans without any health insurance, and to put this in more recognizable format - that is roughly 1 in 6.5 of the population, and this is not even including the underinsured. BTW. This is for a research study - so please vote if you can.
Is the monitoring so far getting us any closer to formulating a policy with a resounding success?
