Is euthanasia acceptable for Australians?

Recently the South Australian government has made a push to legalise doctor assisted suicide (Euthanasia) under strict guidelines.

What guidelines should be followed if a patient decides to make use of euthanasia laws? (More than one can be selected)
The patient must have an incurable disease, leading to death within 12 months.
The patient must have an incurable disease, leading to death within 6 months.
The patient must be a resident of that state. (To prevent international, and interstate people taking advantage of the laws)
2+ witnesses must be present, one of whom is not a relative, to the patient requesting euthanasia. (Excluding the doctor, who must also be present)
A waiting period must be observed between the time of first requesting euthanasia, and confirming the request. (2 weeks - 1 month 'cooling off period')
A second doctor must also believe euthanasia is the best option.
The patient must be sound of mind, and know what will happen in the procedure, and know of all possible treatments available.
