If a spouse declines a truth or dare, is there a penalty or forfeit

Yes - there is a standard forfeit (what)
Yes - there is an 'incremental' forfeit (gets harder each time) (what)
Yes - a forfeit is chosen at random
Sort of - there is no forfeit for the declining the truth/dare, but they 'lose' the game, which has a consequence, which may or may not be a forfeit per-se e.g. winner gets to choose the next date night (describe)
