Yes. If your mother was Jewish, you are Jewish for the rest of your life (according to traditional Judaism), no matter what you do. If you were brought up Christian and wanted to become Jewish, then you wouldn't need to convert if your mother were Jewish since you're already Jewish.
Just so you know, the traditional answer (which Orthodox & Conservative Judaism adheres to) is that a Jew is someone whose mother was Jewish, or who converted into Judaism.
Question 11 asks whether or not you disagree with the statement "Despite an American Jew's personal opinion on the Israeli-Palestinian conflict, he or she should always support the policies of Israel's elected government."
The choices should be "Agree" and "Disagree", without the additional statement that agree means "a Jew should always support Israel" and disagree means "a Jew shouldn't always support Israel". You're asking us to answer two questions at once.
I would say agree with "A Jew should always support Israel", but not with the statement in the question about governmental policies. The Knesset is not the same as Israel; I can support the country without agreeing with its current leaders.
